Subdomain Applacation

RULES!:
1) You must know FTP, i don't have the time to teach you (this also means php includes).
2) You must add me as a friend on myspace, if you haven't already do it now.
3) You must link me as your host on all of your pages!
4) You myst not have an insane amount of advertisements.
5) You must make all of your own content, do not steal or jock from other, it will resort in a deletion of your subdomain.
6) you must contact me when you are going on a vacation or hiatus, or when you quit so i do not delete your site.
7) You must update atleast once every two weeks and put dates on your updates, please.
8) You must use a program like photoshop or psp, if you use something else you have to be amazingly talented at it.
9) You must make your own domain layouts.
10) You must put your site up within 20 days of getting your login information.
11) You must let me know if you want your subdomain deleted.
12) ONLY SUBMIT A FORM ONCE, i get annoyed by multiples and won't pick you.

Subdomain includes:
A url (sitename.lovevoltlayouts.co.cc).
A ftp account (with your desired username and password).
As much space/bandwidth you need.
An advertisement on my site!
